Effective January 1, 2014:
The Office of Entertainment Industry Development will no longer be accepting any transfers to the state. Any and all documentation need to be sent directly to the Louisiana Department of Revenue at the following address:

The template (which can be found here) must be filled out and notarized and all supporting documentation along with the transfer fee must be submitted to LDR.
Until January 1, 2014, OEID will continue to process these transfers; however, please make the transfer fee check payable to the Louisiana Department of Revenue.
